  • Identify satisfaction level of the services using card

Authentication Requirements

  • College head and registrar sign with stamp behind the certificate
  • Signed and stamped document original and photocopy
  • If there is change in name decision letter from court
  • Delegator can delegate someone by sending legal delegation paper from court
